For New Patients: 
Please fill out a New Patient packet, a transferring in Records Release and Consent to treat/policies form.

Once those are filled out, we will be able to get your child built in our system and fax off for their medical records. When we receive your child's records, we will give you a call to schedule an appointment. If your child is ill or has an urgent issue please call the office when these forms are completed at anytime to get scheduled for a visit.

For the Sports Physical Form, the child's legal Guardian/Parent must fill out the first page with the 40 health history questions and get it over to us. Then we can fill out our part and get it back to you provided the patient is Up to date on their yearly physical.
